Connecticut State Requirement for 6th Grade Students

Connecticut State Law requires ALL students to have a physical exam during sixth grade. The CT Health Assessment form needs to be on file in the nurse’s office by the last day of 6th grade. We accept physicals dated from June 1, 2024. Upon completion of the exam, please email the completed CT health assessment record to the lower division health office. New Canaan Cares Youth Advisory Board

Please join the New Canaan CARES Youth Advisory Board. This is a new Board will meet 3 times per year. We look to receive feedback from teens on what issues they are facing today and brainstorm on ways that we can work together to help! We will work on an Ad Campaign with the youth to help educate the community on an issue that teens are facing today!
